I constantly have phlegm in my throat, which I believe is causing halitosis. I regularly drink water all throughout the day, and I scrape my tongue when I brush my teeth, but nothing seems to work...

more detailed history is needed to know the cause(s). However, you might be having chronic sinusitis, post nasal drip or reflux. which cause cough and phlegm.
you need also to check your teeth at a dentist to make sure it's not the cause for your halitosis.
