Can someone die during an operation if they are allergic to anesthetia?

There is no such thing as allergy to Anesthesia.
Some patients have a deficiency in one of the enzymes that is used to breakdown the muscle paralysis drugs. In these cases the patients may remain paralyzed for a long period of time but will recover eventually. There is no effective screening for this enzyme deficiency because it is extremely rare. When appropriately managed it does not lead to death.
Complications can happen during anesthesia. One such example is the entity called malignant hyperthermia where the patient's body reacts to the anesthesia medications. Again if properly managed this should not lead to death.
Other types of complications can occur which are due to physician error however on the whole Anesthesia In 2013 is extremely safe with a very very low rate of complications.
