With Gleason 7 and PSA of 12 the risk of distant metastasis specially to the lymph nodes is around 10%. But the risk of locally extended disease is higher this can be ruled out by the a digital rectal exam and the prostate biopsy.
The bladder tumor is almost always from the bladder and it is caused most of the time from smoking, the biopsy will give us more details about its nature.
