Could someone with high IQ + high achievements to be afflicted with ADHD / ADD? If yes, what would be the telltale signs clinicians look for?

Yes of course he/she can. In fact having a high IQ will delay the diagnosis as the person would find ways to deal with their ADD without affecting their life. Once they enter university
however or start working, high IQ is not protective anymore as the load of work
(or study) increases.
The symptoms we look at are: lack of attention to details, making
careless mistakes, trouble staying focused; easily distracted, appears not to
listen when spoken to, difficulty remembering things and following instructions,
trouble staying organized, planning ahead, and finishing projects. These symptoms
