I have acne on my back and shoulders that are painful when barely touched. I also have black heads on my chest that also became pimples when I tried to remove them. It's been this way for 12 years now
I think you probably have acne and if it is persistent for that long then you should consult your dermatologist to give you a treatment. Most probably, you will get pills for several months that can peel off these lesions unless they are scars. Upon examination more explanations about treatment and expectations can be given based on your case.
