Dr. Rania Mousa General Medicine
Patulous Eustachian tube, also known as patent Eustachian tube, is the name of a physical disorder where the Eustachian tube, which is normally closed, instead stays intermittently open. When this occurs, the patient experiences autophony, the hearing of self-generated sounds. These sounds, such as one's own breathing, voice, and heartbeat, vibrate directly onto the ear drum and can create a "bucket on the head" effect.
This condition is more common in females than in males and is usually present in adolescents and adults
SOME POSSIBLE CAUSES :
>In most instances, patulous eustachian tube is idiopathic.
>Weight loss (sometimes caused by chronic illness) and pregnancy are identified as important predisposing factors.
> History of recurrent otitis media with effusion, with tympanostomy and adenoidectomy is also frequent THIS IS YOUR PREDISPOSING FACTOR OR CAUSE MORE PROBABLY
>Neurologic disorders that may cause muscle atrophy (eg, stroke, multiple sclerosis, motor neuron disease) have been implicated.
>Adhesion formation in the nasopharynx following adenoidectomy or radiotherapy may also predispose individuals to a patulous tube.
>The condition is sometimes associated with medications (eg, oral contraceptives, diuretics). >Other predisposing factors include fatigue, stress, anxiety, exercise, and temporomandibular joint syndrome.
Normally, the Eustachian tube is closed, which helps prevent the inadvertent contamination of the middle ear space by the normal secretions found in the back of the nose. A dysfunctional Eustachian tube that is always open is called a "patulous" Eustachian tube. Patients with this rare condition are plagued by chronic ear infections.
As Eustachian tube function worsens, air pressure in the middle ear falls, and the ear feels full and sounds are perceived as muffled.
>>Some consideration and treatments method:
-To temporarily alleviate symptoms, try positional maneuvers, such as tilting the head to one side or upside down, lie down on their backs, or sit in a chair with their head between their knees. Similarly, a routine of lying down four times per day with legs elevated to around 20 inches for at least two weeks has been attempted as well.
-Recline or lower head when symptoms occur
- removing caffeine from diet, reduce exercise, or gain weight.
-Estrogen (Premarin) nasal drops or saturated potassium iodide have been used to induce edema of the eustachian tube opening.
- Nasal medications containing diluted hydrochloric acid, chlorobutanol, and benzyl alcohol have been reported to be effective in some patients, with few side effects
In extreme cases surgical intervention may attempt to "bulk up" the Eustachian tube tissues with fat, gel foam, or cartilage or scar it closed with cautery
So my advice is to go to an ENT doctor specialist to make the necessary diagnostic tests and measures to exactly diagnose and look for your eustacian tube condition and level of patency.according to your specific condition he will give the appropriate treatment .
Patulous tube is a troublesome but benign condition to be treated, don't hesitate in that .
