Dr. Rania Mousa General Medicine
Since you take isosupra for one year knowing that the usual duration of therapy is about 6 months treatment then you had already have 2 courses of treatment of isosupra and you get fine as you mentioned .

now ,you experience the acne flare ups again ,well it is common for some people but usually isosupra have long lasting effects due to its mechanism then my suggestion is to find if there is a cause for you acne preventing it from total healing rather than taking another course with all its side effects .

My recommendation would be that you check with your dermatologist. A hormonal work up to rule out polycystic ovary disease PCOS or other endocrine disorder might be of value. Perhaps, you could be placed a birth control pill and either topical acne treatment

I would suggest trying a maintenance regimen of topical agents including a retinoid (retin-a, or other), and/or combination benzoyl peroxide/antibiotic product if you are not already using one or consider a hormone-based prescription medications if your acne clearly gets worse around the time of your periods.

It's hard to say without examining you you may have only a slight flare which responds to topical and/or oral anitbiotics. There are some new topical medications, you can ask your dermatologist about it which take away a lot of the redness and inflammation from your recurring acne lesions.

But if topical medications don't control it, and it appears that you'll need another long-term committment to oral antibiotics, then a second 5-month course of isosupra may be added in the eye of your dermatologist
