Doing a freshman year means you're going into an American system University. In such system you need to do 3 more years of premedical studies (you can pick whatever you like asllong as you take all the required premedical courses - they specify them for you in the University. But it'll be much more feasible for later in to go for Biology or medical lab, because they provide courses that are introductory to some of the subjects that will be studied in more depth in medical school). You then sit for the MCAT (medical college acceptance test), and with the acceptable scores on both your premed courses and this test, you get matriculated into med school. So it's a total of 7 years, given that you finish your undergraduate degree in 3 years. The good thing about the American system is that it's based on courses, so if you fail one course you get to repeat it.
If however you want to go straight into medical school after yourffreshman year, then you'll have to switch to a British or French system University. 7 years is the duration. Here if you fail you have to repeat the entire year.
Surgery usually requires 6 years of residency, more if you decide to do a subspecialty (like brain surgery for example).
Good luck!
