Most of the time, ankle sprain is associated with ligament injuries without any fractures noted. So the X ray will be normal. It is very important to treat properly an ankle sprain especially when it happens for the first time. MRI is helpful but elevation to reduce swelling and ankle stabilization with an ankle brace or sometimes with a cast is needed for the injured ligaments to heal.
