Dr. Rania Mousa General Medicine
To treat back pain the first issue is to treat the cause ,the treatment depend also on duration of pain for how long and according to associated symptoms .
if you done all tests and you are seen by your doctor where you don't have any organic cause or mass or disc , X-ray or an MRI are done to find out if something such as a broken bone or a herniated disc is causing your pain.if the doctor said they are normal then no fracture or herniated disc is seen and there is another cause

most common causes are :
>muscle strain, or injury to the muscles from overusing ,that may lead to inflammation of ligaments, and discs that support your spine.
>if you have a poor posture you might have this pain ,try to follow a good posture when standing sitting or sleeping if you want i can give you some tips
> Herniated disc where a pressure on the spinal nervesspinal nerves from certain problems this will appear on an xray or MRI
>A fracture of one of the vertebrae.this will appear on MRI or XRAY
>OsteoarthritisOsteoarthritis caused by the breakdown of cartilage that cushions the small facet joints in the spine.this will show on xray
> Myofascial pain that affects the connective tissue of a muscle or group of muscles
if no serious cause ,you can benefit from physical therapy and you can take proxen for the pain .if your condition now is worse than before consult your doctor again to re do the tests again if something not visible before it might now clear and the cause is known so treatment will be ruled out depending on the cause

You might do the following to minimize your pain :

-keep as active as possible but avoid long standing
-take a hot warm bath when you have a back pain this may ease the pain
-exercise regularly especially stretching exercises ,such as walking, running, swimming, etc. There is no firm evidence to say that any particular back strengthening exercises are more useful to prevent back pain than simply keeping fit and active
-avoid heavy lifting
-lose weight this have big effect on reducing back pain
-stop smoking and alcohol, smoking affects joints and inter-vertebral spaces and affects inflammation healing process
- put a small firm pillow between the knees when sleeping on the side helps to ease symptoms at night
-proxen is fine when taking if it eases your pain if not you will need stronger one but take it with a stomach protective medicine such as gastrimut, pariet ,....
-you can ask your doctor for a muscle relaxant if you feel your back tender and it can be used for short time period
- The doctor may add antidepressant medicine to the pain killer if you suffer chronic unspecific back pain
-physical therapy is of great benefit

So the treatment plan focuses first on conservative treatment and lifestyle if this relieve pain it is ok if not then we add an anti-inflammatory or pain killer if that relieve the pain it is ok if not we add other medicine or procedure depending on the cause ,.....thus proceeding to the next step depending on what step the patient is satisfied

Again the cause is the key for treatment
