Maybe you're straining too hard- Do you suffer from constipation? Or do you usually have hefty dinners close to bedtime? Pain that is triggered by breathing indicates some form of pressure on the phrenic nerve, which originates in the neck and runs down between the lung and heart to reach the diaphragm (the main respiration muscle). This nerve is important for breathing, as it passes motor information to the diaphragm and receives sensory information from it. Any compression against the diaphragm, be it due to bloating, or straining, may produce pain upon breathing.
