The answer is right there: refrain from drinking more than one-two drinks per week, and preferably stick to wine rather than shakes, STOP smoking, and of course follow a healthy diet, meaning no deep fried and junk food! You need to opt for fiber-rich foods, lean meat and fish. And it so happens that just few days ago, on April 22 2015, the British Journal of Sports Medicine released an article entitled "It is time to bust the myth of physical inactivity and obesity: you cannot outrun a bad diet", where they showed that performing 30 min of moderate exercise, five times a week, although more powerful than many drugs administered for chronic disease prevention and management, and despite the fact that regular physical activity reduces the risk of developing cardiovascular disease, type 2 diabetes, dementia and some cancers by at least 30%, it is not enough to balance out a bad choice of diet. The Lancet global burden of disease reports have found that "poor diet now generates more disease than physical inactivity, alcohol and smoking combined. Up to 40% of those with a normal body mass index will harbor metabolic abnormalities typically associated with obesity, which include hypertension, dyslipidemia, non-alcoholic fatty liver disease and cardiovascular disease". So yes you are lean and physically active, which is great, but your diet is not healthy. Start from there and see how it goes. If the desired levels of lipids are not achieved despite managing your diet, you may need to take medications to adjust your lipid levels, especially if you have a familial predisposition for lipid disease.
