congratulations first of all, i do recommend to do serology for Rubella & Toxoplasma in addition to the obligatory labs before marriage ( CBC, HIV ) as i do recall
You should get this form from the "Order of Physicians" (beyt el tabib); some doctors have it in their office (costs 30,000 for the form). It mentions all the tests that need to be done, which are mainly: CBC, blood type (an additional test might be added depending on the CBC), tests for HIV and Hepatitis B (sexually transmitted diseases), in addition to some optional tests (I prefer if you discuss those tests with your regular doctor - I personally recommend some for women, and others depending on "risky sexual behavior").
